Share Your Experience, Shape Better Care

We are looking for patients, clients, caregivers, and community members to share their experiences using health and social services. Your insights help us improve how people access and receive care across our region.

As a member of the Patient, Client, and Caregiver Partnership Council, you will:

  • Bring your lived experience and perspective, including those of vulnerable populations, to guide decisions and co-design programs.

  • Collaborate with the OHT on projects, working groups, and strategic initiatives that impact patient care.

  • Provide valuable recommendations that inform decision-makers and shape the future of healthcare in our community.

Who We’re Looking For

We welcome partners who:

  • Have lived experiences with health or social care, ideally related to one of our priority populations:

    1. Individuals with mental health and substance use challenges

    2. Older adults with complex needs

  • Are not employees, clinicians, or Board members of OHT partner organizations (or immediate family of such employees/clinicians/Board members).

Opportunities to Get Involved

Patient, Client, and Caregiver Partnership Council

  • Attend bi-monthly Council meetings (1.5 hours)

  • Participate in working groups or projects aligned with your interests (1–2 hours per month)

  • Contribute to shaping programs, strategies, and services in the Northern York South Simcoe region
