Seamless Care Optimizing the Patient Experience (SCOPE)

What is SCOPE?

SCOPE is a virtual, interdisciplinary program connecting primary care providers (PCPs) in the community to real-time
supports when they need it most. Through a single point of access, SCOPE quickly connects PCPs to local providers who can help with urgent consultations, diagnostic imaging, curated home and community care support, and navigating community and/or acute services.

Do you have complex patients but don’t know where or how to get them additional services in the community or hospital? Call SCOPE for the Nurse Navigator to assist you with navigation!

Do you need to connect with an Internist for a quick consult to determine the best next step for your patient? Call SCOPE to speak with an internist and/or schedule an appointment for your patient.

 Do you have elderly patients requiring homecare services and/or are at risk of  hospitalization? Call SCOPE to directly get connected with a Home and Community Care Support Services’ coordinator to get assistance with homecare referrals, request additional services if condition has changed and/or assistance with navigation of homecare services.

Helen Poff is our Nurse Navigator!

Helen has over 30 years experience in nursing, and spent 22 of those years building connections at Southlake Regional Health Centre as as Staff Nurse, Charge Nurse and Discharge Planner. In these roles, she extensively collaborated with community partners and primary care.
